Referrals can be made by any current employee or 1099 contractor of Symmetry (must have signed EA/ICA on file)
Leadership and Program Chiefs are unable to qualify for referral bonuses
Referral cannot be working at any current Symmetry or Baylor Scott & White facility
Referral must sign as a full-time employee/contractor
Referring employee must still be employed at the time of the payout
Previous Symmetry Employees/Contractors are not eligible to be a referral
All referrals must be submitted using the form below.

Referral must not be an active candidate in the recruiting process
Good for 6 months from the date of the referral logged in the system
If the same individual candidate is referred by two different Symmetry employees/contractors, we will honor the employee/contractor who submitted their referral first, for a period of six months. After that six- month period expires, if the same candidate is referred again, the employee/contractor who submitted the new referral will receive the bonus.
Symmetry Anesthesia reserves the right to make changes to the referral program at any time.
